1. Agreement to Terms

These Terms of Service ("Terms") constitute a legally binding agreement between you ("you" or "Patient") and MaxLife MD, LLC ("MaxLife MD," "we," "our," or "us"), governing your access to and use of the website at maxlifemdpeptides.com and all related services, including physician consultations, prescription fulfillment, and product delivery (collectively, the "Services").

By accessing our website or initiating a consultation, you confirm that you are at least 18 years of age, have read and understood these Terms, and agree to be bound by them. If you do not agree, please do not use our Services.

2. Nature of Services

MaxLife MD is a telehealth facilitation platform that connects patients with independently licensed physicians for medical consultations. MaxLife MD does not practice medicine. All medical decisions — including whether to prescribe peptide therapy — are made solely by the licensed physicians who conduct your consultation.

If a physician determines that peptide therapy is clinically appropriate for you, your prescription will be dispensed by a U.S.-licensed compounding pharmacy and shipped directly to you. MaxLife MD does not compound, manufacture, or dispense medications.

Important Notice

A physician consultation is required before any prescription can be issued. Completing a consultation does not guarantee that a prescription will be provided. The prescribing physician retains full clinical discretion.

3. Eligibility

To use our Services, you must:

  • Be at least 18 years of age.
  • Be a resident of the United States.
  • Provide accurate, complete, and truthful information during the consultation intake process.
  • Not have a medical condition that a licensed physician determines makes peptide therapy inappropriate for you.
  • Not use the Services for any unlawful purpose.

4. Consultation Process

To receive a prescription, you must complete a medical intake form and participate in a consultation with a licensed physician. During this process, you agree to:

  • Provide complete and accurate health history, including all current medications, supplements, and known allergies.
  • Disclose any conditions that may be relevant to your care.
  • Understand that the physician may decline to prescribe if they determine therapy is not appropriate for you.
  • Follow up with your prescribing physician or your primary care provider as recommended.

MaxLife MD physicians are licensed in the states in which they practice. Our Services are available to patients in all 50 U.S. states, subject to applicable state telehealth regulations.

5. Prescriptions and Pharmacy Fulfillment

All peptides dispensed through MaxLife MD are compounded by U.S.-licensed 503A compounding pharmacies. Compounded medications are not FDA-approved but are legally prepared pursuant to a valid prescription from a licensed physician. Compounded peptides are prepared for individual patient use and are not commercially manufactured drug products.

Prescriptions are valid for the duration specified by your prescribing physician and are subject to renewal consultations. MaxLife MD does not guarantee prescription renewal; each renewal is subject to physician review.

6. Payments, Refunds, and Cancellations

6.1 Consultation Fees

Consultation fees are charged at the time of booking and are non-refundable once the consultation has been completed, regardless of whether a prescription is issued. If a physician determines that therapy is not appropriate for you, the consultation fee is still earned by the physician for their professional time.

6.2 Product Orders

Payment for prescribed products is processed at the time of order confirmation through our secure payment processor (Authorize.net). Because compounded medications are prepared specifically for you, all sales are final once an order has been submitted to the pharmacy for compounding.

6.3 Exceptions

Refunds or replacements may be considered in cases of verified shipping damage, pharmacy dispensing error, or product quality issues. Contact our support team within 7 days of receipt to initiate a review.

6.4 Subscription Programs

If you enroll in a recurring program, you may cancel at any time before the next billing cycle. Cancellations do not apply retroactively to orders already submitted for compounding.

7. Shipping and Delivery

Compounded peptides are shipped directly from the licensed pharmacy to your address on file. Shipping times vary by pharmacy and location. MaxLife MD is not responsible for delays caused by the pharmacy, shipping carrier, or factors outside our control. Products requiring temperature-controlled shipping will be packaged accordingly.

You are responsible for ensuring your shipping address is accurate. We cannot reship or refund orders lost due to an incorrect address provided by you.

8. Medical Disclaimer

The content on this website — including product descriptions, educational materials, and benefit statements — is provided for informational purposes only and does not constitute medical advice, diagnosis, or treatment. All medical decisions are made by licensed physicians during your consultation.

Peptide therapy may not be appropriate for everyone. Results vary by individual. These products have not been evaluated by the U.S. Food and Drug Administration and are not intended to diagnose, treat, cure, or prevent any disease.

9. Prohibited Uses

You agree not to:

  • Provide false or misleading information during the consultation intake process.
  • Attempt to obtain prescriptions for purposes other than your own personal use.
  • Resell, redistribute, or transfer any prescribed products to another person.
  • Use our website in any manner that could damage, disable, or impair its functionality.
  • Attempt to gain unauthorized access to any portion of our systems or data.
  • Use our Services in violation of any applicable federal, state, or local law.

10. Intellectual Property

All content on this website — including text, graphics, logos, product names, and imagery — is the property of MaxLife MD, LLC or its licensors and is protected by applicable copyright, trademark, and intellectual property laws. You may not reproduce, distribute, or create derivative works without our prior written consent.

11. Limitation of Liability

To the fullest extent permitted by law, MaxLife MD, its officers, directors, employees, physicians, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our Services, including but not limited to adverse reactions to prescribed medications, shipping delays, or reliance on website content.

Our total liability to you for any claim arising from these Terms or the Services shall not exceed the amount you paid to MaxLife MD in the 90 days preceding the claim.

12. Indemnification

You agree to indemnify and hold harmless MaxLife MD, its affiliates, physicians, and service providers from any claims, damages, losses, or expenses (including reasonable attorneys' fees) arising from your violation of these Terms, your misuse of the Services, or your provision of false or inaccurate information.

13. Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of Louisiana, without regard to conflict of law principles. Any dispute arising from these Terms or the Services shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association, except that either party may seek injunctive relief in a court of competent jurisdiction to prevent irreparable harm.

14. Changes to These Terms

We reserve the right to modify these Terms at any time. When we do, we will update the "Last Updated" date above. Continued use of our Services after any modification constitutes your acceptance of the revised Terms.
